Format: 4 x 4 plus goalie
Team Fee: $800.00
Includes referee fees
The team listed as the home team must supply 3 pucks
for each game. League will begin in May.

A deposit of $50.00 must accompany your team’s registration form. Deadline to enter a team is May 10, 2011. Players must be at least 16 years of age by June 1, 2011.
